Tej Carbone, LCSW - Founder
Tej Carbone, LCSW is a seasoned clinical operations leader with years of experience in healthcare strategic planning, product development, and cross-functional collaboration. Tej brings a unique blend of clinical and operational skills, specializing in optimizing and scaling integrated behavioral health services within primary care settings. Her expertise lies in clinical operations, specifically in the areas of capacity planning, go-to-market strategies, and product management.
Currently, Tej serves as an advisor for Fort Health, Redesign Health, and several other healthtech startups and medical groups on the topics of building and scaling behavioral health services, with a focus on Collaborative Care (CoCM) implementation and leveraging technology to scale behavioral health services.
In her previous role, Tej was the Director of Strategy and Analytics at Concert Health, a post Series-B Collaborative Care (CoCM) organization. There, she led strategic initiatives across 14 states and oversaw product management and data governance for Clinical Operations, and partnered with the C-suite team to develop tools and strategies to increase profit margins. Prior to that, she was the Director for a multi-state region within Concert Health. There, she built strong relationships with external stakeholders, launching and scaling CoCM services within a large health system that cares for over 6 million patients annually. She also partnered with the system’s Epic analysts to develop tools and workflows for the care delivery and billing of CoCM. Seeing a need to expand CoCM services to pediatric practices, she led a taskforce to develop a pediatric care workflow which was then implemented org-wide.
Tej holds a Master of Social Work and a Bachelor of Social Work from the University of Central Florida, and is a Licensed Clinical Social Worker in several states.
